Dura-arachnoid- Subarachnoid space (Lumbar cistern)
Once outside the brain stem, the CSF enters the subarachnoid space (the space between the arachnoid and pia) and exits into the superior sagittal sinus via specialized structures in the sinus wall called arachnoid villi.
Expanded areas of the subarachnoid space are called cisterns, the largest of which is the lumbar cistern, between vertebrae L2 (where the spinal cord ends) and S2 (Fig. 12).
The CSF flows freely throughout the subarachnoid space; thus a subarachnoid hemorrhage, usually the result of leakage from an aneurysm in the region of Willis, is felt as a severe neckache or backache, reflecting the seepage of blood down to spinal cord regions.
Symptoms in meningitis are similar, because of the diffusion of pus cells down the subarachnoid space.
